We are part of a national drive to learn about people's hospital discharge experiences.

We want to understand how delays in being discharged from hospital affect people, and whether people are leaving hospital safely and with the right support.

What you tell us will help Healthwatch England raise patient and carer experiences with national decision-makers, including the NHS and the Government.

What we are doing

This project will explore both delayed discharge and cases where people felt their discharge was poorly planned, poorly communicated, unsafe, or too early; for example, because the right support, equipment, information, care or follow-up arrangements were not in place.
