Current overview of Service 

Date of Assessment: 18 March to 27 March 2025. The service is a residential home providing support to older people living with dementia, nursing needs, mental health conditions and physical disabilities. This assessment was to follow up on previous concerns we had identified at our last inspection. We had also received some information of concern about the service. Previously we were concerned about managing peoples risks and ineffective governance. At this inspection we have identified a continued breach in relation to risk management and governance. We have also identified breaches in relation to safeguarding and person-centred care. The governance systems in place had failed to identify and rectify the concerns found at this assessment. Whilst a new manager was in post and was implementing new systems and processes, these required embedding into practice at the time of our assessment.

 

People's experience of the service

We received mixed feedback from people and relatives about the service. Relatives gave us mixed views about if they felt their relatives were safe living at the home. One relative said, “They care about my relative. They are definitely safe there.” Another said, “I would like to say she is safe, but I have my doubts. ”People told us staff were kind and caring. Comments included, “It’s all fine to me. They are all really kind to my relative. They all know her. When they [staff] talk to [relative] they smile at them [staff]. My relative talks about them affectionately. I would recommend it.” Another relative said, “Staff interact with my relative very well. They like a joke and a bit of banter; they do laugh and joke with him. They interact with him well.” Relatives said they felt the home was clean and tidy. Relatives gave us mixed feedback about people’s appearance and clothes. Comments included, “My relative is now clean and tidy and clean shaven” and “My relative always looks clean and well cared for.” However, other relatives commented, “My relative always looks like they don’t belong to anyone especially in other people’s clothes” and “I do get asked to take clothes in for [relative] but they never have them on, sometimes they have other people’s clothes on. ”Relatives said there was always enough staff. Comments included “There’s always plenty of staff around when I have visited,” “There’s enough staff “and “I think there is plenty of staff around.”While the people we spoke with expressed they were generally happy with their care, our assessment found elements of care did not always meet the expected standards.

Current overview of Service 

We carried out this announced on-site inspection on 8 April 2025.

We found the practice had met regulations.

Infection control procedures in place were effective.

The practice had systems to identify and manage risks. On the inspection day, we identified areas needing improvement. The provider acted promptly and submitted evidence showing the actions taken.

Recruitment procedures reflected current legislation and there was effective leadership and a culture of continuous improvement.

Patients’ care and treatment was provided in line with current guidance.

Patients were treated with dignity and respect and at the time of our inspection, could access care, support and treatment when required.

The practice ensured equipment was safe to use and maintained and serviced according to manufacturers’ instructions.

 

Grimethorpe and Shafton Dental Practice is part of TSJ Smile Limited, a dental group provider. The practice is in Barnsley and provides mainly NHS dental care and treatment for adults and children.

There was step-free access to the practice and car parking spaces available, including dedicated parking for disabled people, at the practice.

The practice had 4 treatment rooms. At the time of our inspection, there was a total of 13 staff, of which there were 3 dentists, 3 dental nurses, 2 dental therapists, 2 receptionists, 2 administrators and the practice manager. We gathered feedback from staff and spoke to a range of staff during our inspection, including 1 dentist, 2 dental nurses, 1 dental therapist, 1 receptionist, 1 administrator and the practice manager.

 

Peoples experience of the service

On the day of our inspection, we saw patient feedback from 10 patients. Patient feedback provided a positive view of the dental team and care provided by the practice.

Patients commented positively about the standards of cleanliness.

Patients felt able to book appointments within an acceptable timescale for their needs and said they had enough time during their appointment without feeling rushed.

 

Patients told us they were given clear information to help them make an informed choice about their treatment and any associated costs. They were involved in decisions about their care.

 

Patients told us that when they were prescribed medicines, sufficient information was given.

 

Patients told us that they were supported to maintain their oral health and were provided with appropriate information and resources.

 

Patients told us they would benefit from having information on how to access emergency dental care when the practice was closed.

 

The practice shared patient feedback with the team. We were told this was reviewed and where suggestions had been made, appropriate action would be taken.
